Knowledge record — {record-id}: {title}

One material claim/decision and its completed or explicitly unresolved disposition. Presence is not acceptance. Fill applicable fields; reasoned N/A only when inapplicable.

Field Value
Record identity Stable locally allocated ID; no global allocator
Publication intent Exact source claim/checkpoint and intended effect; retain on equal retry
Kind human knowledge / technical-reference decision
Statement Complete material claim or decision
Applicability Exact project, actor, condition, time and scope
Grounds and uncertainty Evidence, rationale, limitations and what remains unknown
Disposition observed / accepted / retain-only / rejected / superseded / unresolved
Source Exact locator, claim/heading and inspected epoch; Git object/path or preserved evidence
Originating task and unit Actual source task, phase/checkpoint and native producer address
Producer Actual author of this record, distinct from original source when copied
Qualifier Existing responsible role/principal and actual working unit
Acceptance authority Actual owner/valid grant and exact decision reference, or explicitly absent
Completed effect or missing decision Actual publication/reuse/retention; exact owed action and existing owner
Relations Kind, target identity/epoch, scope and grounds; explicit none after lookup

Source and decision context

Preserve enough context to assess the claim without inventing unseen conversation or copying every source section. Human-only knowledge retains human provenance; technical findings retain technical sources. Two paraphrases of one origin are not independent corroboration.

Relations and current use

Relations may be equivalence/duplicate, scoped successor, correction or unresolved conflict. A legacy target names its exact path, D/F/heading identity and source epoch. Preserve old meaning; a scoped successor replaces only that scope. Search knowledge/records/ for incoming references to this identity and material ancestors before use; follow relevant chains in both directions. A newer timestamp does not resolve overlapping conflict.

Retry and continuation

Compare actual source, material intent, effect and acceptance. Equal retry reuses completed output; divergent intent refuses replacement until an authorized correction resolves it. Retain-only states why no publication/resolution is owed; unavailable acceptance-critical context keeps close open.
