Phase B integrated the Evidence concept into TFW's three lifecycle workflows: handoff.md (new Step 11 "Collect evidence" between build gate and Pre-RF Gate, step renumbering 11-12 → 12-13, §5 Evidence in mandatory sections), review.md (2 new Trust Protocol entries for evidence claims, Step 3 evidence audit reference), plan.md (3-line coordinator reminder for Evidence fields in Step 7), and RF.md (TD-1 fix from Phase A). 4 workflow files modified + README Task Board. Scope well-bounded, no overreach.

4. Verdict¶
✅ APPROVE
Phase B delivers what the TS specified: Evidence lifecycle is now active end-to-end across coordinator (plan.md reminder) → executor (handoff.md Step 11) → reviewer (review.md Trust Protocol + Step 3 reference). All 5 ACs met, all 7 HL principles enforced. The work is precise — Step 11 matches existing step density, Trust Protocol entries follow the established pattern, and the plan.md reminder is a pointer without duplication.
No discrepancies found. TD-1 from Phase A is fixed. The two observations (plan.md numbering confusion, glossary stale ref) are pre-existing issues properly routed to Phase C.
